• We will never use your data for advertising or share it with advertisers.
• You can request deletion of your data at any time.
• We will tell you clearly when something changes.

2. Information We Collect

We collect information in two ways: information you give us directly, and information generated by your use of the platform.

3. How We Use Your Information

We use the information we collect to:

• Create and maintain your account
• Operate the consent-based introduction process between Providers and Recruiting Partners
• Send you platform notifications, updates, and support communications
• Improve platform functionality and user experience
• Maintain platform security and prevent fraud or misuse
• Comply with legal obligations

We do not use your data to serve advertisements. We do not build advertising profiles. We do not share your data with data brokers.

4. Provider Privacy — How We Protect You

Provider privacy is the defining feature of PhysDent. Here is exactly how it works:

• Your real name, email, phone number, NPI, and contact details are never visible to Recruiting Partners on the platform.
• You are represented by a stealth alias until you choose to connect.
• Before any introduction occurs, you are shown the full identity of the Recruiting Partner — their name, organization, and role — so you can make an informed decision.
• Your profile is never indexed by search engines.
• You can pause your profile, hide it from all Recruiting Partners, or delete it entirely at any time.
• We maintain an audit log of every introduction request made in connection with your profile.

5. PhysDent Staffing, LLC — Affiliated Entity Outreach

PhysDent Platform, LLC has an affiliated entity, PhysDent Staffing, LLC, which connects opted-in Providers with short-term, locum tenens, and contract staffing opportunities sourced from third-party healthcare organizations and staffing companies.

6. Sharing Your Information

We do not sell your personal information. We share information only in the following limited circumstances:

• With Recruiting Partners: Only after you explicitly approve an introduction. At that point, we share only the information necessary to facilitate the connection — your name and contact details. We do not share your full profile data unless you choose to provide it.
• With service providers: We work with third-party vendors who help us operate the platform (such as cloud infrastructure and email delivery). These vendors are contractually prohibited from using your data for any purpose other than providing services to us.
• With PhysDent Staffing, LLC: If you have opted into the staffing program, your information may be shared internally with PhysDent Staffing, LLC for the sole purpose of identifying relevant opportunities. PhysDent Staffing, LLC is subject to the same privacy commitments described in this policy.
• For legal compliance: We may disclose information if required by law, court order, or valid legal process. We will notify you of such requests where legally permitted to do so.

7. Data Retention

We retain your information for as long as your account is active. If you delete your account:

• Your profile is immediately removed from all Recruiting Partner views.
• Your personally identifiable information is deleted within 30 days of your request.
• Aggregate, de-identified analytics data may be retained indefinitely as it cannot be linked back to you.
• We may retain certain records as required by law or for legitimate business purposes such as resolving disputes.

8. Cookies and Analytics

We use cookies and similar technologies to operate the platform, maintain your session, and understand how the platform is used.

We do not currently use third-party advertising cookies or tracking pixels. If we add analytics tools in the future, we will update this policy and disclose specifically what data is collected and how it is used. We will never allow analytics tools to access your personally identifiable information.

9. Security

We take data security seriously. Our practices include:

• All data in transit is encrypted using TLS
• Data at rest is encrypted
• Access to user data is role-based and audited
• We conduct regular security reviews

No system is perfectly secure. If we become aware of a security breach affecting your data, we will notify you promptly in accordance with applicable law.

10. HIPAA Notice

PhysDent and PhysDent Staffing, LLC are not HIPAA-covered entities. We do not collect, store, or transmit protected health information (PHI) as defined under the Health Insurance Portability and Accountability Act. The platform is a career navigation tool, not a clinical or healthcare system.

Please do not submit any patient health information through the platform. If you believe PHI has been submitted in error, contact us immediately at privacy@physdent.com.

11. Children's Privacy

The platform is intended for licensed healthcare professionals and adults 18 years of age and older. We do not knowingly collect information from anyone under 18. If we become aware that a minor has created an account, we will delete it promptly.

12. Your Rights

You have the right to:

• Access the personal information we hold about you
• Correct inaccurate information
• Request deletion of your data
• Opt out of staffing outreach at any time
• Export your data in a portable format
• Lodge a complaint with a data protection authority

13. Changes to This Policy

We may update this Privacy Policy from time to time. When we do, we will update the effective date at the top and notify registered users by email. If a change materially affects how we handle your data, we will provide at least 14 days notice before it takes effect.

We will never retroactively change how we use data you have already provided without your explicit consent.
